What to Bring to an Emergency Dental Appointment in Hamilton
A 2-minute check before an urgent dental visit can save time at the desk and help the dental team review your information more quickly. If you are coming in for emergency dental care in Hamilton, bring the items below if you have them. If you cannot find everything, still come for care.
Bring these if you can
- Photo ID such as a driver’s licence, Ontario photo card, passport, or another government-issued ID.
- Dental coverage details including any dental card, plan information, or member number.
- A current list of medications, plus vitamins and supplements. A note on your phone is fine if that is easiest.
- Your allergy list, including drug allergies and any reactions you have had in the past.
- Any recovered tooth or fragment if a tooth was broken or knocked out and you were able to find it.
If a tooth broke or came out
If you recovered the tooth or a broken piece, bring it with you. The tooth or fragment may still be useful for the dental team to examine, even if it cannot be put back in place. If you are not sure whether something counts as a fragment, bring it anyway.
If you do not have everything
Do not delay urgent dental care just to gather paperwork. Come in with what you have. The office can often help sort out missing details when you arrive.
